The primary objective of a customer questionnaire format is to find out what customers think about a particular company and their products and services. This type of questionnaire is normally created according to a specific format and can contain several questions, with each question focusing on a particular aspect of customer thinking and perception.

This question needs to ask the customer how willing they are to try new products of the company and what they would like to suggest to the company for improvement in product or service quality. ]
