Padi Medical Questionnaire


A Padi medical questionnaire is usually used by a physician to examine an individual participating in scuba diving. It helps the physician to assess whether the individual is fit for scuba diving or not. Getting a positive response from this questionnaire does not prevent the individual from taking part in scuba diving but it rather makes him aware that there are health hazards while scuba diving.
