Job Performance Questionnaire


A job performance questionnaire is primarily designed to evaluate an employee’s performance. It provides the employer with the information he or she requires to assess and correct an employee’s performance. The relationship between the two is of mutual give and take, and thus such a questionnaire is necessary for both to improve their communication. It essentially conveys the employee’s feedback.
