Fun Questionnaire Format

A fun questionnaire format needs to be created in such a way that it integrates and retains the fun element in every question. The basic format of a fun questionnaire remains the same although you can change the type of questions that can be asked and vary between multiple choice and polar questions.

Sample Fun Questionnaire Format:

Name: ____________________________ [Name of the participant]

Address:__________________________ [Home Address of the participant]

Gender: _____________________ [Gender of the participant (M/F)]

Age:______________________________ [Age of the participant]

Email: __________________________________ [Email address of the participant]

First Question: [The First question should be framed in such a way that it breaks the ice and gives a clear indication of the questions to come. The first question should start the ball rolling or set the pace for the questionnaire. Most appropriate questions would be: which is your favorite horror movie or how high can you jump etc]

Second Question: [The second question should focus further on the fun part of it and can be simple, weird, wacky, or just rhetorical. Questions like: who would you like to spend a week in a remote island or how you would react if your house was on fire are perfect.]

Third Question: [The third category of questions can be on any specific area like travel, actors/actresses, bachelors, colors, flowers, Tv shows etc. The idea is to make the entire exercise funny and at the same time relaxing.]

Fourth Question: [The fourth and final category of questions should nail the idea of fun deeper. Questions like what character role would you like to play in a Sci-Fi movie or what would you want to be in your next life are appropriate for the questionnaire.]
