Employee Self Assessment Questionnaire

An employee self assessment questionnaire is usually given by companies to their employees before an appraisal, so that the employees can answer questions about their own performance and do an evaluation of what they have achieved in a particular period of time. This leads to a constructive discussion between the employer and employee regarding the latter’s performance.
